Tips/Gratuities

How did you tip? In cash or did you charge your tips to your passenger account?
Was your fare "all inclusive" with no requirement to leave an extra gratuity? But did you tip extra anyway?
Was there "automatic tipping", i.e., was a pre-set amount which was deducted from your passenger account?
Did you tip in the "old style" by handing envelopes with cash to your cabin steward, waiters, etc.?
